Duties Include...

1. Manages a participant caseload including communication with participants, providers and family or authorized representatives and care plan development. Reviews member claims histories and identifies intervention opportunities. Contacts and interviews members to conduct a baseline assessment, assess self-care ability, assess knowledge and adherence deficits, develop a holistic care plan, and set goals. Uses evidence-based protocols, develops and implements interventions to achieve care plan objectives. Assesses effectiveness of treatment plan and progress toward goals, and modifies individualized care plans based on outcomes and acuity. Educates members about accessing services, in-network use, national guidelines for care, and self-management skills and strategies

2. Collaborates with primary care physician and other treating professionals to develop member goals, create a care plan, and track adherence to clinical guidelines. Develops physician summary reports that include information about the members health status, self-care adherence information, depression screen results, medication utilization, education received and any social service or other referrals.

3. Employs active listening and motivational interviewing techniques to build relationships with members and their authorized representatives. Performs care management across the continuum of care. Encourages participants to participate in their health care decisions and assists member with researching treatment options in order to communicate effectively with providers and to make informed decisions.

Education Requirements

Requires a Bachelors degree. Additional years of qualifying work experience may be considered in lieu of Bachelors degree.

Experience Requirements

Require a current Missouri or Kansas RN license in good standing, and a minimum of two years of recent RN case management or disease management experience in a hospital or managed care setting, such as a health insurance environment and at least two years of nursing experience in a clinical setting. Requires current knowledge of clinical standards of care and disease processes. Also requires critical thinking skills, effective verbal and written communications skills to consult with physicians and providers, and demonstrated ability to negotiate costs, ability to use a personal computer, and knowledge of medical information systems.
